A prescription is a small set of numbers where a single digit changes the product entirely. It is transcribed from paper into an order and then travels to a lab.
Verification before the lab, not after
Every check after dispatch is a remake. A second look before the order goes costs seconds and is the only point where the error is still free.
Cause is the useful record
Remakes recorded as remakes tell you nothing. Split into transcription, lab error, fitting and customer adaptation, the pattern points at a specific fix, and transcription is usually the largest.
